Physical Therapist License

Michigan license to engage in the unrestricted practice of physical therapy under Part 178 of the Public Health Code.

Licensing authority: Michigan Board of Physical Therapy (Department of Licensing and Regulatory Affairs, Bureau of Professional Licensing) · Application information

Physical therapist licensure by endorsement (R 338.7136)
Application
Completed application and required fee on the department form (online via MiPLUS).
Board Page Jurisprudence Examination
The Board's Physical Therapy Examination page states: "In addition to the NPTE, all applicants for physical therapist licensure must take and pass the jurisprudence examination before licensure can be issued." and "Licensure candidates must take the jurisprudence examination through PSI." The Board's current rules (R 338.7132 and R 338.7133, amended in 2026 MR 5, effective March 5, 2026) adopt only the NPTE for physical therapists, and the Physical Therapist Licensing Guide (Revision Date 1/26/2026) lists no jurisprudence examination; the administrative rule outranks the page, so the route is complete on the rule's terms and the page's demand is shown here so the applicant sees what the Board page states it will ask for.
Canadian Education
If licensed in a province of Canada, completion of the educational requirements in Canada or in the United States for licensure as a physical therapist (R 338.7136(1)(c)).
Canadian Examination Alternative
R 338.7136(1)(d)(ii) accepts, instead of the NPTE, a passing score on the Canadian Alliance of Physiotherapy Regulators Physiotherapy Competency Examination taken for a license in a province of Canada; that is another country's national examination and is recorded as a condition.
General Requirements
Requirements of the Public Health Code and rules applying to all applicants: good moral character, criminal background check with fingerprints (MCL 333.16174), one-time human trafficking training (R 338.7126), implicit bias training, working knowledge of English, and verification of every license ever held elsewhere showing no pending discipline or sanctions in force. R 338.7136(2) requires disclosure of every other license and verification under MCL 333.16174(2).
License Elsewhere
A current and full physical therapist license in another state or in a province of Canada (R 338.7136(1)(b)).
Physical therapist licensure by examination (R 338.7132)
Application
Completed application and required fee on the department form (online via MiPLUS).
