Licensing authority: Georgia Department of Community Health · Application information

Identity Documents
Supply government-issued social security card and photo ID matching the registry identity; provide name-change documentation as necessary.
Medical Availability
Candidates on medical light duty may take the written/oral exam, but may not take the skills evaluation until medically released to full duty; contact the provider for authorization.
Prior Certification
Active unencumbered nurse aide registration in another U.S. jurisdiction, without pending or substantiated abuse, neglect or property-misappropriation findings.

4. The Georgia Nurse Aide Registry will verify registry status in the state that currently lists your name as a certified nurse aide. Upon state verification, the Georgia Nurse Aide Registry will evaluate the complete application, along with the state verification information before adding your name to the Georgia registry without taking the Georgia Written/Oral and Skills Competency Examination. You must meet all the eligibility requirements listed in Part I.
