Licenses, application pathways and examination requirements
Each license and applicant pathway has its own requirements and exemptions. Choosing practice questions does not determine your eligibility.
Certified Phlebotomy Technician (CPT)
Licensing authority: National Healthcareer Association
Initial by examination
- Clinical
- CPT candidates must document at least 30 successful venipunctures and 10 capillary or finger sticks on live individuals, subject to the handbook's pre-externship timing provision.
- Education
- Full certification requires a high school diploma or equivalent; provisional certification is outside this task.
- Employer Training
- The employer must complete an NHA agreement allowing the exam after verified coursework and before required on-the-job training. NHA reports pass or fail but withholds the certificate number and certificate from the candidate, employer, and regulatory or state agency until it receives notice of completed training. The candidate has 180 days from the exam date to finish the on-the-job training and ensure the employer notifies NHA of completion of the entire program. If the candidate does not finish on time or the employer fails to notify NHA, NHA voids a passing score and requires the candidate to retake the exam for certification.
- Pre Externship
- The institution must complete an NHA agreement allowing the exam after verified coursework and before its required externship. NHA reports pass or fail but withholds the certificate number and certificate from the candidate, employer, and regulatory or state agency until it receives notice of completed externship. The candidate has 90 days from the exam date to finish the externship, ensure the institution has the necessary program-completion paperwork, and have it notify NHA. If the candidate does not finish on time or the institution fails to notify NHA within the applicable time, NHA voids a passing score and requires the candidate to retake the exam for certification.
- Training Or Experience
- Pathway 1 requires qualifying training within the past five years through an accredited or state-recognized program, registered apprenticeship, military medical services training, employer training, or pre-externship coursework. Training completed five years or more before registration does not qualify under Pathway 1. Alternatively, Pathway 2 requires one year of supervised experience in the last three years or two years in the last five years in the field covered by the exam.
