Licenses, application pathways and examination requirements
Each license and applicant pathway has its own requirements and exemptions. Choosing practice questions does not determine your eligibility.
Massage Therapist License
Individual license under the Massage Therapy Practice Act. Original issuance, reciprocity, military pathways, renewal, reactivation and reinstatement are routes of this license. The pre-May 1, 2017 transitional issuance window is closed. Probation under OAC 175:20-5-1(c) is a condition on this license, not a separately named credential.
Licensing authority: Oklahoma State Board of Cosmetology and Barbering · Application information
Discretionary reinstatement after suspension or revocation

- Prior status
- License suspended or revoked.
- Waiting period
- Apply after thirty days.

- Renewal fees
- 59 O.S. 4100.6(C) permits renewal without dues or fees during deployment.

Expedited full license endorsement for a military spouse
At least one of the following alternatives:
- Applicant group
- Licensed military-spouse applicant meeting 59 O.S. 4100.5(A); subsection B additionally specifies active-duty spouse, transfer to Oklahoma and leaving employment elsewhere to accompany the spouse.
- Application
- Submit Form 321 selecting Reciprocity and the $115 fee.
- Background report
- Provide a criminal history report from the jurisdiction where licensed, dated no more than thirty days before application and including sex offender and violent offender searches; if unavailable there, provide an OSBI report.
- Board application alternative
- The Board licensing page states: "Test scores showing completion and passing the Massage and Bodywork Licensing Examination (MBLEx) or the National Certification Board for Therapeutic Massage and Bodywork (NCBTMB) examination". Form 321 also names NCBTMB without identifying a particular examination. OAC 175:20-5-1(a)(5) expressly names MBLEx for original licensure. The discontinued NCBTMB licensure examinations may supply historical score evidence. The state sources do not identify BCETMB as the NCBTMB examination they accept, so the issuer-only wording is not equated with the frozen BCETMB version.
- Board review
- The Board may deny or impose probation for substantially related crimes or reasonable public-safety threats, unprofessional conduct, violations, or prior revocation or discipline. OAC 175:20-5-7(e) bars eligibility for three years after a plea or conviction for a listed crime; later applications are evaluated individually.
- Criminal and disciplinary disclosure
- Disclose criminal pleas or convictions other than minor traffic violations and revocation or discipline elsewhere; attach certified felony court records as Form 321 requires.
- Education
- Show educational and examination requirements equal to or exceeding Oklahoma requirements; the Board may require validation of documents by a recognized agency at applicant expense.
- Expedited endorsement
- The Board expedites endorsement and issues the full license when it finds the prior jurisdiction's requirements substantially equivalent under 59 O.S. 4100.5(C)(1).
- Lawful presence
- Attach the affidavit verifying lawful presence required by Form 321.
- Liability insurance
- Provide evidence of current professional liability insurance for massage therapy.
- Military fees
- For active-duty personnel and spouses covered by 59 O.S. 4100.8(D), application and first-period license fees are waived.
- Minimum age
- At least eighteen years old; submit birth certificate, driver license or other government identification.
- Other examination evidence
- Another state's own examination or an unidentified examination used for previous licensing is evidence of equivalent qualifications, not an Oklahoma exam leaf.
- Photograph
- Provide a full-face photograph approximately 2 by 2 inches, taken within the last year.
- Prior license
- Provide official certification or affidavit of licensing history and completed hours from the licensing authority, with seal and official signature, in a sealed envelope or already on file. License copies are insufficient.
- Score report
- Form 321 names MBLEx as a passing-score option. Existing equivalent examination qualifications are subject to recognition under 59 O.S. 4200.9(A) and OAC 175:20-5-2(a)(3); the reference does not automatically require a new sitting.
- Testing
- 59 O.S. 4100.7 does not automatically remove testing. MBLEx is the identified ordinary exam; equivalence of an existing score and any need for another sitting remain Board determinations.
- Universal recognition scope
- 59 O.S. 4150.1(B)(1) requires no Oklahoma statutory reciprocity authority for the discipline; 59 O.S. 4200.9(A) already supplies massage therapy reciprocity authority. This is the profession-specific reciprocity route.
Expedited full license issuance following military transfer or discharge
At least one of the following alternatives:
- Advance application
- May apply before actual transfer or discharge under 59 O.S. 4100.8(A).

- Testing
- Military provisions do not automatically dispense with testing. MBLEx is the identified ordinary examination; the Board determines equivalence of existing examination qualifications and remaining requirements.
