Lead-Based Paint Supervisor Certification
DEQ certification to supervise lead-based paint abatement services in target housing and child-occupied facilities. The Department administers the registered EPA national supervisor certification examination.
Licensing authority: Oklahoma Department of Environmental Quality · Application information
Annual renewal, including the statutory thirty day grace period
- Application
- Submit a complete renewal application, applicable renewal fees, refresher certificate, previous-year quarterly reports and Oklahoma Tax Commission compliance. The ordinary renewal filing period is January 15 through March 31, with a thirty-day grace period through April 30 without late fees.
- Compliance
- Meet OAC 252:110-7-5 renewal criteria, including cooperation in compliance inspections, compliance with the laws and certificate conditions, and payment of money owed. A timely complete renewal application extends the authorization until DEQ issues or denies renewal.
- Prior status
- Hold a current supervisor certificate; the first renewal of a military reciprocal certificate follows its separate conversion route.
- Refresher training
- Complete eight hours of DEQ-accredited supervisor refresher training in the preceding 12 months. Required training assessments are course-completion conditions.

New initial certification after expiration beyond the reactivation window
- Accredited initial training
- Successfully complete DEQ-accredited initial supervisor training (at least 32 hours including at least 8 hands-on hours), and submit its completion certificate within six months; OAC 252:110-11-3, 252:110-9-1.1 and incorporated 40 CFR 745.225(c)(6)(iii).
- Application
- Submit a complete, signed certification application, education and experience documentation, required notarized citizenship and general affidavits, and applicable certification and exam fees; government-work fee exemption requires the prescribed affidavit. DEQ contacts the applicant after the application is complete to schedule testing.
- Certification period
- Initial certification runs from approved issuance through the following March 31, or the requested next full certification year April 1 through March 31; a training certificate is not permission to begin work before DEQ certification.
- Course assessment
- The accredited training provider assesses hands-on skills and administers a course test, or a proficiency test, under incorporated 40 CFR 745.225(c)(7). These provider-specific course completion assessments are training conditions, not an identified separately registered certification examination.
- Examination time limit
- Under incorporated 40 CFR 745.226(b)(6)-(7), at most three examination attempts within six months of the initial course completion certificate; retake the accredited course before reapplying if certification is not obtained within that period.
- Experience
- One year as a certified lead-based paint abatement worker, or at least two years of experience in a related field or the building trades; incorporated 40 CFR 745.226(b)(1)(iii)(C).
- Instructor restriction
- OAC 252:110-11-1(c) bars initial or renewal certification based on a training certificate from a course the applicant conducted as instructor or participated in as guest speaker.
- Prior status
- Hold an expired and unreactivated supervisor certificate beyond the one-year reactivation window; 27A O.S. 2-12-301(F) requires applying and qualifying for a new certificate and the DEQ program page redirects to initial certification.
Reactivation or reinstatement of an expired certificate within one year
- Conflicting application instruction
- 27A O.S. 2-12-301(E) makes this reactivation depend on renewal requirements, and the DEQ program page lists no certification exam. Form 110-301 page 4 instead places Initial and Reinstatement under the instruction "(Must select Exam Fee)". The higher-ranked statute controls this route; the application instruction is retained for the applicant to address with DEQ.
- Documents
- Provide a completed reinstatement application, initial-level fees, quarterly reports for the inactive and previous certification years, OTC compliance and a request-for-reinstatement letter. The applicant affidavit states that no LBP services were performed while uncertified.
- Period
- Reactivation runs from DEQ approval through March 31, followed by annual renewal.
- Prior status
- Expired supervisor certificate; apply within one year of its March 31 renewal date. Beyond one year use new initial certification.
- Refresher training
- Submit the supervisor refresher-course completion certificate, valid for 12 months, and satisfy applicable education, experience and renewal requirements.

Renewal or continuation during military deployment and after discharge
- Agency discretion
- 59 O.S. 4100.6 authorizes DEQ to keep the certificate in good standing during active duty, waive fees and continuing-education or other renewal acts in the stated deployment circumstances, and continue it for at least one year after discharge. Request applicable waivers and provide the military-duty documentation; this is not a new initial reciprocal certification.
- Prior status
- Already hold an Oklahoma supervisor certificate in good standing at military activation.

- Instructor restriction
- OAC 252:110-11-1(c) bars initial or renewal certification based on a training certificate from a course the applicant conducted as instructor or participated in as guest speaker.
- Military training credit
- Under 59 O.S. 4100.4(A), DEQ must accept satisfactory equivalent military education, training and experience and apply it favorably toward certification or approval for examination. Under 59 O.S. 4100.7 this does not automatically waive testing or other eligibility requirements.
First renewal or conversion from military reciprocal certification

- Hold a DEQ military reciprocal supervisor certificate approaching its first expiry; DEQ requires conversion using an initial application, rather than the ordinary refresher-only renewal.
- Submission window
- DEQ Form 110-307 Guidance page 3 requires a discipline-specific initial course within six months before application and completed initial application and fees within 30 days of reciprocal certificate expiry, together with passage of the discipline-specific LBP certification exam.
Military or military spouse reciprocal certification
- Affidavit instruction
- Form 110-307 guidance step 4 calls its Citizenship Affidavit Form 110-304; the current initial application identifies the citizenship affidavit as Form 002-028. Preserve the official instructions and obtain the applicable form from DEQ.
- Eligibility
- Active-duty military personnel or their spouse with a current and valid comparable LBP supervisor certification, receiving transfer or honorable discharge orders to Oklahoma. Apply in advance with existing certification, orders or DD214 and the requested supporting affidavit documentation.
- Equivalency
- DEQ determines good standing and reasonable equivalence and accepts the prior training and certification under OAC 252:110-11-7.1(b), issuing the reciprocal Oklahoma certification within 30 days. Initial application and first issuance fees are waived.
- First renewal
- DEQ Form 110-307 Reciprocity Guidance page 3 requires an initial supervisor course and the LBP certification exam before expiry of the reciprocal certificate. That is represented as the separate first-renewal/conversion route.
- Period
- OAC 252:110-11-7.1(c) states a minimum one-year period unless DEQ identifies cause for denial or missing documentation. 59 O.S. 4100.8(C) states the same period as full certification; DEQ guidance uses the March 31 certification schedule. Applicants must follow the period DEQ places on their certificate.

- Prior temporary certificate
- Hold a temporary military supervisor certificate; submit the missing documents, satisfy the DEQ equivalency determination or obtain a successful appeal. The subsequent first renewal is the separately recorded initial-course and EPA-examination route.

- Instructor restriction
- OAC 252:110-11-1(c) bars initial or renewal certification based on a training certificate from a course the applicant conducted as instructor or participated in as guest speaker.
- Prior action
- After at least one year from denial, revocation or suspension, apply for a new supervisor certification under 27A O.S. 2-12-304(A)(2); new issuance rests in DEQ discretion and all applicable initial qualifications must be satisfied. Reinstatement of a suspended existing certification, as distinct from issuance of a new one, is retained as a secondary-route note.
Initial certification using a current out of state or agency certification
- Accredited initial training
- Hold a currently valid lead-based paint certification from another state or certifying agency. Request case-by-case equivalency review; DEQ may waive initial training but not the DEQ-accredited supervisor refresher course under OAC 252:110-11-7. Submit certification, completed training and other services, the issuer requirements and program contact, and the $500 nonrefundable equivalency-review fee.
- Examination waiver
- 27A O.S. 2-12-301(B) permits DEQ to waive testing, training, experience or examination prerequisites when the issuer requirements are equivalent and reciprocal recognition is available. This is discretionary: the current DEQ FAQ expressly requires passing the third-party EPA exam for this pathway. The examination remains a leaf.
Certification under the Universal Licensing Recognition Act
- Initial training credit
- DEQ must apply substantially similar and verifiable experience, education, training and supervision favorably under 59 O.S. 4150.1(B)(3); the ordinary initial-training conditions apply insofar as not satisfied by DEQ-approved equivalent evidence.
- Instructor restriction
- OAC 252:110-11-1(c) bars initial or renewal certification based on a training certificate from a course the applicant conducted as instructor or participated in as guest speaker.
- Prior examination
- 59 O.S. 4150.1(B)(4) permits proof of an equivalent or substantially similar examination from another state or the Oklahoma examination requirement as DEQ determines. An examination belonging to another state is a condition; a passing score on the currently offered registered EPA supervisor exam remains an examination reference regardless of when it was taken. DEQ rules and its current supervisor exam instructions identify the Oklahoma requirement as the EPA supervisor certification exam.
- Recognition eligibility
- Under 59 O.S. 4150.1(A)-(B), move to and establish physical residency in Oklahoma (or qualify as an accompanying military spouse not applying under the military Act); hold a comparable out-of-state credential in good standing for at least one year, meet substantially similar qualifications, and have no conflict with a compact, reciprocity or equivalency agreement as DEQ determines. Provide work, education, training and any supervision evidence, required fees and disciplinary-status disclosures. This is conditional recognition, not automatic issuance.
