Licenses, application pathways and examination requirements

Each license and applicant pathway has its own requirements and exemptions. Choosing practice questions does not determine your eligibility.

Lead-Based Paint Activities License — Supervisor (Project Supervisor)

Supervisor is the licensed discipline named in IC 16-41-39.8 and 410 IAC 32; the application calls it Project Supervisor. The EPA-developed third-party examination references the frozen national supervisor exam. Provider-developed course tests are training conditions; the additional department examination is an unnamed discretionary slot. For the Title 25 military-board definition, this survey reads IDOH as continuation of the State Board of Health founded in 1881, whose 1992 change the official history describes as a reorganization. IC 25-1-17-1 points to IC 25-1-8-1 and IC 25-0.5-9; section 37 of that enumerating chapter covers other occupational or professional agencies created after June 30, 1981. Under this continuity reading those general military-board provisions do not establish additional pathways for this Title 16 supervisor credential.

Initial licensure by examination
Additional department examination
The discretionary examination slot is unnamed and remains a condition. 410 IAC 32-2-4(d) states: (d) In addition to the requirements of subsections (a) through (b), the department may require an applicant or a designated representative of a contractor, in the case of subsection (b), to take an examination administered by the department. The examination shall cover only the discipline for which the applicant is seeking a license. The commissioner shall deny the application if the applicant does not receive a passing score of seventy percent (70%). If the department denies the application, the certificate of training is invalid and the applicant must retake and pass the initial training course for the discipline for which the applicant is seeking a license and any subsequent third-party examination.
Application
Submit State Form 49240 or the available online application, required training and experience documentation, proof of examination passage, and two recent identifying photographs required by 410 IAC 32-2-4(e).
Course assessments
Pass the provider-developed written supervisor course test, with a minimum passing score of 70 percent, and the applicable hands-on assessment. Providers develop their examinations from their submitted blueprints (410 IAC 32-3-5). These are training conditions rather than one identified uniform licensing examination.
Exam eligibility and attempts
Complete the course, obtain the certificate, and meet the experience requirements before examination. At most three third-party examination attempts are permitted within six months of the certificate. Failure to pass within six months requires repeating initial training before reapplying (410 IAC 32-2-3(d)-(f)).
Experience
Have one year of experience as a licensed lead-based paint abatement worker OR two years in a related field including lead, asbestos, environmental remediation, or construction trades (410 IAC 32-2-3(b)(4)).
Fee
Pay the nonrefundable $150 application fee under 410 IAC 32-2-9(a).
Prior passage
Prior discipline-examination passage from an EPA-approved program is accepted subject to the training-lapse rule. State Form 50748 states: 3. Fill out this form only if you are applying for INITIAL lead license(s) and only if you want to take the third-party exam(s) in Indiana. (If you have already taken the third-party exam(s) for your license discipline from an EPA-approved state or tribe lead program, you do not have to take the exam(s) again unless the item listed next applies to your situation.) 4. If you have allowed more than forty-eight (48) months to lapse since your last training course(s), you will have to take initial training course(s) again and also take the third-party exam(s) again. 5. If you are renewing lead license(s), this form is not necessary unless you have a lapse in training as stated in the previous item above.
Rules awareness
If the approved discipline course is not Indiana-approved, attend an Indiana-approved two-hour rules-awareness course within twelve months before application (410 IAC 32-2-3(a)(2)).
Training
Successfully complete an approved supervisor initial course of at least 32 training hours including eight hours of hands-on training, and obtain the certificate (410 IAC 32-2-3(a), 32-3-3(3)).
Initial licensure by reciprocity
Additional department examination
The additional discretionary department examination is an unnamed slot. 410 IAC 32-2-7(d) states: (d) In addition to the requirements of subsections (a) through (b), the department may require an applicant or a designated representative to take an examination administered by the department. The examination shall cover only the discipline for which the applicant is seeking licensure. The commissioner shall deny the application if the applicant does not receive a passing score of seventy percent (70%). If the department denies the application, the certificate of training is invalid and the applicant must retake and pass the initial training course for the discipline for which the applicant is seeking a license and any subsequent third-party examinations.
Application
Complete Parts A, B, C, G and I of State Form 49240, attach the license, examination and rules-awareness evidence, sign, and provide the two identifying photographs.
Exam retake exception
Fee
Pay the nonrefundable $150 application fee under 410 IAC 32-2-9(a).
Previous exam passage
Provide proof of having passed the supervisor third-party examination. Passage before application remains a requirement leaf for this currently offered examination.
Qualifying license
Hold a current supervisor license from a U.S. EPA-authorized state or tribal lead program and provide copies of all current lead-based paint licenses (410 IAC 32-2-7(a)).
Rules awareness

Renewal without an extended training lapse
Application
Submit a completed renewal application, refresher certificate, proof of third-party examination passage, and the two recent identifying color photographs under 410 IAC 32-2-5(a), (c).
Board page instructions
The rule controls the twelve-month license and refresher windows. The lower-ranked current IDOH Contractors page instructs applicants as follows: To renew a license, the applicant must possess a license in the discipline up for renewal which has been valid within the previous six (6) months.  The applicant must successfully complete approved refresher training. For an Inspector, Risk Assessor, Supervisor, or Worker, the refresher training includes a minimum of 8 hours within the previous 36 months.  A minimum of 4 hours of refresher training within the previous 36 months is required for a Project Designer. A license may not be renewed in the instance that 48 months have elapsed between training courses for the same discipline
Course assessment
Provide the refresher certificate showing course completion and passing the provider written course examination (410 IAC 32-2-5(a)(4)(A)).
Fee
Pay the nonrefundable $150 application fee under 410 IAC 32-2-9(a).
License recency
Have held a valid supervisor license within the previous twelve months (410 IAC 32-2-5(a)(1)).
Previous exam passage
Have taken and passed the supervisor third-party examination and provide proof under 410 IAC 32-2-5(a)(3)-(4); this currently offered examination remains a leaf although ordinary renewal uses prior passage.
Refresher training
Complete approved supervisor refresher training within the previous twelve months under 410 IAC 32-2-5(a)(2), lasting at least eight hours under 410 IAC 32-3-4(2).
Retake exception
New examination registration is required for a qualifying training lapse.
Training continuity
The forty-eight-month training-lapse provision in 410 IAC 32-2-5(e) requires initial training and the third-party test; that pathway is retained separately.
Relicensure with initial retraining and reexamination after an extended training lapse
Additional department examination
For initial applications, the unnamed discretionary department examination condition in 410 IAC 32-2-4(d) applies: (d) In addition to the requirements of subsections (a) through (b), the department may require an applicant or a designated representative of a contractor, in the case of subsection (b), to take an examination administered by the department.
If seeking renewal, also meet the prior-license window in 410 IAC 32-2-5(a)(1).
Exam retake
Take the supervisor third-party examination again; prior passage does not remove the applicable training-lapse reexamination requirement.
Implementation lapse wording
State Form 50748 uses more than forty-eight months since the last course. The applicable rule controls its threshold and between-courses wording. The form states: 3.
Initial training
Successfully complete an approved supervisor initial course of at least 32 training hours including eight hours of hands-on training, and obtain the certificate (410 IAC 32-2-3(a), 32-3-3(3)).
Rules awareness
If the approved discipline course is not Indiana-approved, attend an Indiana-approved two-hour rules-awareness course within twelve months before application (410 IAC 32-2-3(a)(2)).
Training lapse
For renewal, 410 IAC 32-2-5(e) uses a forty-eight-month lapse between two courses in the same discipline. For licensure generally, 410 IAC 32-2-3(g) says more than forty-eight months. Apply the threshold of the provision governing the application.
