Licensing authority: The Fiber Optic Association, Inc. · Application information

Initial by examination
Language
The direct application process is English-only and requires adequate English skills.
Minimum Age
Applicants must have reached the applicable age of majority, as required by the general FOA certification policy; no uniform nationwide numeric age is substituted.
Prerequisite
CFOT certification is required for CFOS/O; the more general CFOT-or-CPCT skills-category text does not replace this specific prerequisite. Existing CFOT status is a qualification, not an instruction to sit a new CFOT examination in this task.
Qualification Alternatives
[{"path":"FOA-approved school","requirements":"Complete the applicable specialist training, including hands-on splicing and testing; satisfy the KSA requirements and pass the specialist examination. The instructor evaluates hands-on abilities."},{"path":"Direct certification based on field experience","requirements":"CFOT plus at least two years of recent relevant field experience (the linked direct application specifies a minimum of two years of current experience in fiber optics), including documented OSP installation and testing; participation in multiple installation jobs, preferably buried, underground and aerial; OSP cable preparation; fusion splicing; VFL, OLTS and OTDR testing. Employer/manufacturer/vendor training can count toward experience. FOA reviews experience and references and may request further documentation or an interview. Pass the corresponding Fiber U Certificate of Completion exam as a preparation condition before the FOA certification exam; it is not another new certification examination."}]
Scheduling
Application and identity/confidentiality agreements precede the exam. Direct candidates require acceptance and advisor directions/link when prepared, then take the monitored, timed, closed-book exam online. General FOA guidance also allows individual online or paper arrangements with a local proctor who may be a supervisor, instructor, teacher or other professional but not a co-worker or relative; the direct program specifically requires online delivery. No fixed examination window is asserted.
Scope Boundary
Only the CFOS/O knowledge certification examination is submitted. Skills/experience and prerequisite certification remain conditions. Fiber U completion, CFOT, withdrawn CFOS/C and other specialist directions are not newly registered or produced.
Version Applicability
Current official certification-specific KSA and policy pages are the controlling publication on the evaluation date. No named edition, future scope start, expiry or retirement is stated for CFOS/O; unpublished start/end dates remain null.
