- Background Check
- Have the designated vendor send criminal-background-check results directly to the Committee.
- Certification
- Current BACB BCaBA certification in good standing.
- Certification Verification
- The May 2025 application says the administrative office requests BACB certification verification directly; the applicant requests it only if the office cannot locate it.
- Character References

- Historic DIDDOption
- The May 2025 application still lists DIDD qualification before July 12, 2012. T.C.A. 63-11-307(c)(5) requires BACB certification and receipt of a regular license by July 1, 2019; that deadline is applied as closing new provisional issuance. Historical DIDD qualification does not substitute for the current certification requirements of renewal, reinstatement or reactivation.

- Translations
- When necessary, submit both the original document and an issuing-source-certified English translation.
Order of Compliance — Lifting Suspension or Probation
- Appearance
- Appear before the Committee when required; the Board may require an appearance before granting the order. An appearance is not an independently identified examination.
- Effective Restoration
- Restoration requires an Order of Compliance and cannot take effect before the original disciplinary expiration date or the petition date.
- License Status
- T.C.A. 63-11-310(b) separately requires immediate license forfeiture when national certification is lost; that situation is not cured merely by lifting probation or suspension.
- Order Compliance
- Prove compliance with every condition of the disciplinary order, including civil penalties, required continuing education and administrative costs; submit the Order of Compliance petition and supporting evidence.
- Prior License
- An existing license is subject to suspension, probation or a disciplinary condition; this route restores the previously issued license rather than issuing a new license.
